Glucose in circulation can attach to haemoglobin inside red blood cells through a non-enzymatic process. Because red cells remain in circulation for weeks to months, HbA1c reflects an average-like exposure over time, weighted more toward the more recent period. It does not display daily peaks, lows, or timing.
Reports may use a percentage or SI units, and interpretation depends on the laboratory and clinical context. Conditions that change red-cell lifespan, haemoglobin variants, blood loss, transfusion, pregnancy, kidney function, or some laboratory interferences can affect how well the value represents glucose exposure.
HbA1c differs from fasting glucose, fasting insulin, and continuous glucose monitoring. Fasting tests capture a defined moment, while a glucose sensor provides a time series in interstitial fluid. Each measurement emphasizes a different part of glucose regulation, and disagreement can be informative rather than automatically meaning one result is wrong.
The result is sometimes translated into an estimated average glucose, but that conversion is a population-derived relationship. Individual glucose patterns can vary around it. The original HbA1c value and unit are more direct representations of what the laboratory measured.
Because glycation accumulates during a red blood cell's time in circulation, the result is influenced by both glucose exposure and red-cell turnover. This explains why HbA1c can diverge from directly measured glucose patterns in some physiological or laboratory contexts.
