Lipoprotein(a)

Lipoprotein(a), abbreviated Lp(a), is an ApoB-containing lipoprotein particle with an additional apolipoprotein(a) component attached.

Lp(a) resembles an LDL particle but has a distinctive apolipoprotein(a) protein linked to ApoB. Blood levels are strongly influenced by inherited variation and can differ widely between people. The size of the apolipoprotein(a) component also varies, which contributes to measurement complexity.

Laboratories may report mass concentration or molar particle concentration. These units cannot be converted with one reliable universal factor because particle composition and protein size vary. The assay, units, and laboratory reference information should therefore accompany any comparison.

Lp(a) is considered a cardiovascular risk-related marker, but the concentration is not a direct image of the arteries and does not determine an individual outcome. ApoB includes the Lp(a) particle contribution along with other ApoB particles, while an Lp(a) assay focuses on this specific class. The two values answer related but different questions.

Because inherited factors have a strong influence, Lp(a) may be relatively stable within an individual, but measurements still carry biological and analytic variation. Acute circumstances and assay design can matter. A result should be documented rather than recalled from an approximate verbal summary.

Apolipoprotein(a) occurs in multiple inherited size forms, called isoforms. Isoform variation can influence assay behaviour, particularly for mass-based measurements, so method design is part of the reason molar and mass results should remain in their original units.
